Excessive oil in the combustion chamber. Valve seals for leaking.
Perform a compression test.
Combustion chambers for excessive carbon build-up. Remove carbon with top engine cleaner.
Proper camshaft timing.
Incorrect basic engine parts such as cam, heads, pistons, etc.
